Athletic Registration

Athletic Transfers - CIF

If you are a transfer from another high school to Simi Valley High School you will need to contact the Athletic Director to complete paperwork to file as a CIF transfer. 

Students who are CIF transfers may not participate in any SVHS athletic programs until cleared by CIF.

Athletic Clearance

Our office will accept athletic registration paperwork for the 2024-2025 school year in the ASB office beginning May 1, 2024. Please do not send the paperwork via email. 

The Athletic Packet Includes:

1. A completed physical on the SVUSD physical form. 

  • A physical is good for one calendar year from the date the physical was completed, not signed by a doctor. 

  • All incoming 9th graders must complete a physical before they try out for a team; this includes all teams who have summer try-outs. 

  • Must be completed by a doctor (MD or DO) or a physician assisted under the direction of an MD or DO. If physicals are completed at an Urgent Care it must be completed and signed by an MD or DO. This is a required CIF bylaw and district requirement. 

  • Hard copies must be submitted to the Athletics Office with the athletic clearance paperwork. 

  • SVUSD will now accept electronic signatures from Kaiser Permanente. In addition, The SVUSD physical form must be completed and signed by the parent of the athlete.

2. A printed copy of the confirmation form populated from for the 2024-2025 school year

All athletes are cleared through our online system called Athletic Clearance: If you are a first time user you will be required to create an account. At the end of the registration process you will be emailed a copy of the confirmation form. If your athlete is a multi-sport athlete, please complete and return confirmation forms for each sport. This process must be completed every school year. 3. Copy of your athlete's insurance

If your family does not have insurance or in between insurance providers please review the "student accident insurance" section. 

Student Accident Insurance

Per the California Education Code, each student participating in athletic programs (including band, cheer, etc.,) must provide proof of medical insurance for accidental injuries. If a student needs coverage to participate, he/she may purchase coverage through Pacific Educators.

All plans are effective when envelopes are postmarked. It is best to drop off the checks/money orders with the application to the Athletics Office. We will make a copy, and drop in the mail, that way your player can participate on that same day since we know it was completed.

Driver Form - Must be completed to drive student athletes to and from athletic events including tournaments. Driver forms are good for one school year. The driver form must be turned in to the ASB office with a copy of vehicle registration, license, and insurance (including limits).
