Athletic Clearance

All student-athletes must complete athletic clearance each school year. Incoming student-athletes and students who are not currently cleared may not participate in tryouts, practices, or athletic programs at Simi Valley High School until the athletic clearance process is complete. Returning winter and spring sport athletes should follow current Athletics Office timelines for submitting updated paperwork.

Families are encouraged to watch the Athletic Registration Help Video before starting the process.

To complete athletic clearance for the 2026–2027 school year:

1. Complete the student-athlete’s sports physical.
2. Register online through Athletic Clearance / Home Campus.
3. Gather the required documents.
4. Submit the completed documents to the appropriate office.
5. Wait for clearance confirmation before participating.

What Families Need to Complete Athletic Clearance

The Athletic Clearance Packet includes three required documents:

SVUSD Physical / Medical Eligibility Form
A completed sports physical is required before a student may participate in athletics. The physical must be completed on the SVUSD athletic physical form and signed by an MD or DO.

If the physical is completed by a nurse practitioner, physician assistant, or other health care professional, an MD or DO must also sign the form before it is submitted.

Families with Kaiser insurance may submit the Kaiser electronic physical form. The SVUSD athletic physical form must still be completed by the parent/guardian and included with the submission.

Athletic Clearance / Home Campus Confirmation Form
Families must register online through Athletic Clearance / Home Campus. Registration must be completed every school year, even if the student participated in athletics during a previous school year.

At the end of the registration process, Home Campus will generate a confirmation page. The confirmation page must be printed and signed by both the parent/guardian and student.

If a student is participating in more than one sport, families must complete the clearance process for each sport.

Proof of Medical Insurance
Families must provide a copy of the student-athlete’s health insurance card. If a family does not currently have health insurance, student accident insurance information is available on the SVHS Athletics website.

Submitting Documents

Once all three required documents are complete, families should submit the athletic clearance packet to the appropriate office.

During the summer, documents should be turned in to the Front Office.

During the school year, documents should be turned in to the ASB/Athletics Office.

After the Athletic Director reviews and approves the required documents, the student’s clearance will be processed in Home Campus. Families will receive an automated email from Home Campus once the student-athlete has been officially cleared to participate.

Please do not email athletic registration paperwork unless directed by the Athletics Office.

Student Accident Insurance

California Education Code requires each student participating in athletic programs, including band and cheer, to provide proof of medical insurance for accidental injuries.

Families who need coverage may purchase student accident insurance through Pacific Educators. Families who purchase coverage should provide proof of purchase as part of the athletic clearance process.

Athletic Transfers - CIF

Students transferring from another high school to Simi Valley High School must contact the Athletic Director to complete CIF transfer paperwork.

Students who are CIF transfers may not participate in any SVHS athletic programs until cleared by CIF.

Other Athletic Forms

Driver Form
This form is required for adults who will drive student-athletes to or from athletic events, including tournaments. Driver forms are valid for one school year. The completed form must be submitted with a copy of the driver’s license, vehicle registration, and insurance, including coverage limits.

Drug Testing Information Flier
This flier provides information from SVUSD about the voluntary drug testing enrollment program.

Volunteer Drug Enrollment Form
This form is used to enroll a student in the voluntary drug testing program. Completed forms should be turned in to the Main Office.
